As summer is coming to a close, all 7th grade parents, please think of this form when you are visiting the doctor for physicals, or check-ups and ask that he or she fills this out so you will have one less piece of paperwork to worry about at the beginning of the year. Please note that the state of Ohio requires the information below for all incoming 7th grade students. Please use this form and have your 7th grade student bring it to school with them to turn in to the school nurse. Thank you for your cooperation.
